Co-Curricular Participation Information
Students involved in athletic programs are visible representatives of Oak Park and River Forest High. It is crucial that a strong link between good citizenship and opportunities for participation be maintained.
Prospective athletic participants should refer to the Athletic Handbook to become familiar with the terms and conditions that are applicable throughout the school year whether or not the student is in season.
How to Register for Sports at OPRF
Complete the Athletic Participation Form for 2016-2017. A new form must be completed each sport season. Signatures are required for both the athlete and the parent/guardian.
Get your annual physical exam. The best date for a physical exam is after May 1 so it will not expire during the next school year (the physical exam is current for 395 days). All returning athletes will need to submit the IHSA Pre-Participation Examination form or have a physical on file that covers them through the entire season of competition. All freshman athletes can use their school entry physical to serve as the athletic physical if it is dated after May 1, 2016.
Hand deliver the Athletic Participation Form and IHSA Pre-Participation Examination form to the Athletic Office during the appropriate Registration Dates for your sport (see below).
