Location: OPRF Stadium (unless otherwise noted)
Registration: Several weeks prior to the tryout please submit a copy of a recent physical to the school nurse (If you already have participated in a sport this school year, please check to see that your physical is still up to date) and complete the Spring Student Extracurricular Registration Application (forms located outside the Athletic Office) and turn it into the Athletic Office. Once these two requirements are completed your name will be added to the eligible tryout list. Students who register closer to the tryout date might be issued a Blue Card because the coach has already picked-up the list of eligible student-athletes, please turn the Blue Card into your tryout coach and your name will be added to the eligible tryout list. Confirming with the Athletic Department that all information is complete a few weeks before the scheduled tryout is highly recommended.

Student Extracurricular Registration Form Please drop off in the Athletic Directors Office (708.434.3600) or mail to Lori Heifloff, Athletic Office, 201 N. Scoville Street, Oak Park IL 60302. (If mailing please call to confirm receipt the week prior to tryouts and make a copy for your records before sending.)

Physical Form Please turn in a valid physical to the Athletic Office. The physical must be current and last the student athlete through the entire season.
Equipment: Please bring all protective equipment, cleats & running shoes, swim attire, water, a white t-shirt which can have a number written on it, and proper attire for all weather conditions. If you need to see the trainer for any issues please do so prior to the tryout time.
