GUIDELINES for completion of the “Certificate of Child Health Examination”

INCOMING FRESHMEN
Physicals must be returned to Health Services by June 30th.  Your student will not be allowed to receive a schedule, participate on Freshmen day, August, 17th or begin school on August 24th, without a physical. It is important that you schedule an appointment with your primary care physician as soon as possible in order to meet the necessary deadline.  Physical forms may be obtained on the OPRFHS website (click on link below), from the Health Service office or the Welcome Center of the high school.
